What do we do?

Duke Home Health (DHH) provides home health services for adult and pediatric patients needing nursing, rehabilitation (including physical, occupational and speech language pathology therapies) and home health assistance. When medically necessary, our services help patients adapt and recover from illness or injury in the comfort of their home.
 
Our JCAHO accredited home health services are designed to meet the needs of the patient while she/he remains in his or her home. With today's shorter hospital stays, many patients benefit from home health services, which may include nursing services, rehabilitation services and/or social work.
 
In collaboration with the patient's physician, an individualized plan of care is developed for each patient allowing them to remain at home and maintain their independence while attending to their medical conditions.
 

Our Services include:

Nursing Care

Our RN's and LPN's provide direct nursing care and work with patients and their physicians to develop an appropriate and comprehensive plan of care. Specialized nursing services include wound and ostomy care, and infusion therapy.

Home Health Aides

Nursing sssistants provide help with personal care and light housekeeping in conjunction with nursing and therapy services.

Rehabilitative Therapy

Rehabilitative services are provided by our physical, occupational and speech language pathology therapists. Our therapists educate and assist patients to regain their maximum physical capabilities and achieve their highest level of independence. They may assist patients in day-to-day activities and educate them on creating a safe home environment. Speech therapists will assist patients in re-establishing and regaining their speaking ability.

Social Workers

Our social workers assist patients and families in identifying appropriate community resources that may assist with their recovery and ability to remain in their home.
